Leadership Team
Marcia Morgan Parker, Director of Clinical Operations
Marcia has been a key member of the ACCM team since its founding in 2013, ensuring that the organization continues to innovate and that all programming conforms to evidence-based best practices. She earned a Diploma in Nursing from The University Hospital of the West Indies; a M.S. in Nursing with a specialization in Nursing Informatics from Walden University, and; a Doctorate in Nursing Practice from Chamberlain University in Chicago. As part of her doctoral research, Dr. Morgan Parker studied collaboration between RNs and Community Health Workers (CHWs) in performing home care visits as a means to reduce 30-day hospital readmission rates and unplanned ED visits. Her career as a clinician and healthcare manager has been dedicated to creating coaching and mentoring programs, facilitating high quality, reliable systems of care, and developing the next generation of nurses, and health care professionals. Marcia loves listening to music, traveling, and volunteering in her community.
Amber Chen, Senior Social Work Care Manager
Amber graduated from New York University. She is a LMSW and a Certified Case Manager. In addition to providing culturally-sensitive direct services to clients, Amber also provides training and support to new-hires. In her leisure time, Amber enjoys hiking and playing with her cat, and is a huge Lego fan!
Liyuan Wang, Quality Assurance Manager
Liyuan is a LMSW and a Certified Case Manager (CCM), with many years of experience working within care management companies, hospitals, and mental health clinics. She was formally educated at Earlham College and Bryn Mawr College and received further clinical training at the University of Pennsylvania. Liyuan enjoys spending time with family and friends and traveling to experience various cultures, foods and ways of life. She also enjoys swimming and cooking.
Angel Huang, HR/Office Manager
Angel brings six years of management and HR experience to ACCM and ensures that our talented staff operate efficiently, and as a team. Her sound advice and systems management skills helps the company’s managers and front-line team members stay connected to each other, access relevant data and feel supported. In addition to a Bachelor’s Degree in Management, Angel holds a Human Resources Management certificate from Cornell University which equips her to support the long-term success of ACCM and connect ACCM’s staff to foster a healthy work environment. Angel enjoys crafting and decorating, often exploring Pinterest for design ideas, and treasures precious moments with her family - watching movies, taking walks, reading and cooking together.
Natalia Senko, Quality Assurance Manager
Natalia Senko is a registered nurse with a master’s degree in nursing and over 17 years of experience in various fields dealing with compliance, clinical and care management. As a QA Manager who provides day-to-day support to all care managers, Natalia has exerted a strong, approachable leadership style and receives much appreciation from the workforce. Natalia also enjoys learning and exploring to gain both explicit and tacit knowledge.
Care Management Team
Our highly experienced team of 8 Case Managers, 31 Licensed Master Social Workers (LMSW), 32 Registered Nurses (RN), and 34 Care Manager Associates is responsible for ensuring that the needs of all patients are met in a professional, client-centric manner. Our LMSWs/ LCSWs, and RNs have years of experience working with diverse populations and are available 24/7 to remotely assist patients, as required. Our number one objective is caring for people who have chronic illnesses or mental health conditions; no matter how intense their condition may be, our goal is to provide the highest level of care so that they can maintain their lives in the safety and comfort of their own homes.
Based on the directives from our institutional clients, and programmatic goals, our Care Managers maintain contact with patients on prescribed intervals - sometimes monthly, sometimes weekly, sometimes daily. All engagement is provided in the patient’s language of choice, by a team that consists of a nurse, a social worker and clerical staff; currently ACCM has staff that are proficient in English, Spanish and Chinese. Because we understand the languages and cultures of our patients, we are better prepared to understand and serve their individual needs. During these telephonic-encounters we may answer questions related to medication-management, navigating their health insurance, nutrition, exercise, etc. - all related to that patient’s individual care plan. And, when necessary, consultation with a LMSW/LCSW or RN is arranged. But we also do more. Our Care Managers are trained in Motivational Interviewing techniques so they acquire all relevant information from a client and fully understand their issues and concerns. They are compassionate and over time get to know and truly care about our patients. In the end, this leads to better health outcomes, more efficient care and peace-of-mind for our patients, our institutional clients, and our caregiving team.
Community Health Workers (CHW)
Not every problem can be solved remotely, which is why ACCM’s team includes 24 Community Health Workers. ACCM hires CHWs who hail from those communities which they are asked to work. CHWs play a critical role in discrete chronic care management programs such as the Asthma Home-Based Environmental Program.
During the pandemic, all field work for community health workers has converted to telephone care management delivery, which has been very smooth under our streamlined remote working model.
