Asthma Care-Management
As a partner in the OneCity Health Delivery System Reform Incentive Program (DSRIP) Asthma Home-Based Environmental Program since 2017, ACCM has managed asthma care (including pest management referrals) for over 450 pediatric patients through seven NYC hospitals; and we are currently accepting new hospital-clients.
Our team provides community-based education, health monitoring, and coordination of pest-mitigation services in order to ensure adults and children with Asthma remain in good health and avoid unnecessary visits to the ED. Through close-coordination with providers - via the hospital’s EMR system - ACCM care managers follow-up with provider-referred patients directly, reducing the burden on the healthcare institution/provider and guaranteeing patients suffering from Asthma responsive services 24 hours per day.
In 2018/2019 ACCM has been the top-performer among all contracted providers for this program; as reported by NYC Health + Hospitals in its Q3 (2018) Asthma Program Update, ACCM’s rate (245% of expected results) for Completed Home Assessment was more than double that of the provider in the number 2 position.
